Introduction
Many organizations fail to learn from their mistakes and near misses, some organizations even fail to learn from their accidents. Accidents/incidents occur when hazards escape detection during preventive measures, such as a job or process safety analysis when hazards are not obvious, or as the result of combinations of circumstances that were difficult to foresee. A thorough incident investigation may identify previously overlooked physical, environmental, process hazards, or management systems, the need for new or more extensive safety training, or unsafe work practices. TapRooT® is a systematic process, software, and training for finding the real root causes of problems. It is used by leading companies around the world to investigate and fix the root causes of major accidents, everyday incidents, minor near-misses, and others.
Objectives
- An expert system that leads investigators beyond their current knowledge:
- TapRoot® Software (either web-based or individual user) to make the system more efficient and to create a database of investigation results.
- Initiating and conducting an investigation.
- Different skills and techniques of accidents investigation and root cause analysis
- Application of causal factor and root cause analysis for investigating process and non-process incidents.
- Developing appropriate recommendations to address the root causes at various levels to avoid future incidents.
- An intervening technique to obtain facts about the accident.
- Structuring and writing accident investigation technical reports effectively

Training Methodology
This is an interactive course. There will be open question and answer sessions, regular group exercises and activities, videos, case studies, and presentations on best practice. Participants will have the opportunity to share with the facilitator and other participants on what works well and not so well for them, as well as work on issues from their own organizations. The online course is conducted online using MS-Teams/ClickMeeting.
